How do i do a double page spread for a childrens book?

This may sound really stupid but how would you paint a double page spread for a childrens book that is to be published. Would i draw the two pages together side by side on one sheet of paper. Or should i draw both pages on to seperate pieces of paper????

My Out-of-Control Child
You just make the illustration as a single image, using the proportions applicable to a two page spread.
The publisher will arrange for the split during his pagination process.
