David, since no one’s come up with a title for the book you’re looking for and your question’s about to expire I would suggest that you try the following website for locating your book:
It’s a pretty excellent book search site. You simply give them the info that you have on the book you’re looking for and they’ll try to find it for you. They’re a really free service but you do have to register there with an email address. I’ve used them about 4 times so far and they’ve found my books 3 out of the 4 so not terrible at all.
